If you believe that your content was mistakenly removed or disabled as a result of our error or misidentification, you may submit a counter-notification to our designated DMCA agent. Your counter-notification must include the following information:
- Identify the content that was removed or disabled and provide enough information so that we may locate the material.
- Provide your contact information, including your name, address, telephone number, and email address.
- Include a statement that you have a good faith belief that the content was removed or disabled as a result of our error or misidentification.
- Include a statement that you consent to the jurisdiction of the Federal District Court for the judicial district in which your address is located or, if your address is outside of the USA, for any judicial district in which Unified Lifestyles may be found and that you will accept service of process from the person who provided the DMCA notice or an agent of such person.
- Provide your physical or electronic signature.
Please note that we will not take any action on a counter-notification unless we receive evidence that the party who filed the original DMCA notice has filed an action seeking a court order to restrain you from engaging in infringing activity related to the removed or disabled content.
